Re-enrollment packet (9 pages) contains:
Enrollment policy, Financial Responsibility Notice, Acceptable Computer / Internet Policy, School/Parent/Student Compact, Student Income form and
Emergency Medical form

New Students
Please click on each form ( 1 & 2) below to download and print.
After printing and filling out the forms, bring the forms
with the "What parents must bring with the application"
(see below in red) to our Digital Offices.

Application Contains: Application Checklist
Parent/Guardian acknowledgement, Student application form,
Parent / Guardian Information, Special Needs Information,
& State of Ohio management information request.

(FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT)

What parents must bring with the application:
++ Birth Certificate ++
++ Social Security Card ++
++ Immunization History ++
++ Proof of Custody, if applicable ++
++ Proof of Residency ++
(ex. current bill w/address, pay stub, rent receipt or cable bill)
